Privacy Policy
The University Centers of Excellence Medical Offices and Central California Faculty Medical Group (hereafter referred to collectively as “CCFMG”) are committed to protecting the privacy and accuracy of your personally identifiable information. Other than as required by laws that require disclosure, personally identifiable information is not disclosed without your authorization.
In accordance with the California Consumer Privacy Act, Consumer Rights Requests can be made by filling out this form, Consumer Rights Request.
This privacy policy (“Policy”) covers www.inspirehealth.org (the “Site”) is applicable to all information about you (“you” or “your”) collected through the Site. When you access or use the Site, you consent to the collection, use, processing, and disclosure of your information in accordance with this Policy by CCFMG, and its parent, subsidiaries and other entities under common control with CCFMG (“Affiliates”). If you do not agree to the terms of this Policy, you are not authorized to use the Site. All references in this Policy to the Site include other websites Affiliates may create, own and/or operate. This Policy describes the types of information that CCFMG gathers from you and the other people who visit the Site, how CCFMG may use that information and how you can contact us regarding this Policy and your personal information. This Policy also describes additional rights that California Users have under the California “Shine the Light” law and the California Consumer Privacy Act (“CCPA”).
Information Collection and Use
Your information may be collected and used for the following purposes:
- Identify website visitors who have accepted the Personal information and Cookie Policy;
- Identify website visitors logging in to any of the services available on the Site;
- Provide content to website visitors on the Site, including content tailored to you;
- Provide access to CCFMG‘s services or information about CCFMG’s services, such as appointment times;
- Track Internet traffic and visitor use of the Site and particular webpages;
- Track aggregate or individual usage trends of features or services on CCFMG website;
- Provide content via a content distribution network;
- Help administer and protect the security of the Site and provide communication and other services
- Process or complete transactions, such as online bill pay, that you request, including providing your information to Affiliates and other third parties to help process or complete the transactions;
- Track your use of the Site, including which webpages you visit, which links you click, your mouse or cursor placement, and the time intervals between each of these actions;
- Sharing your information to vendors and contractors who provide services to CCFMG;
- Archive or back up the Site or the information for archival, backup, or disaster preparedness purposes;
- Develop and/or enhance products or services by CCFMG Affiliates or third parties;
- Contact you regarding customer service and administrative issues, such as questions about a specific request, or otherwise respond to your comments or requests;
- Send you emails with information and offers from CCFMG or CCFMG Affiliates;
- Maintain suppression or opt-out lists that CCFMG may share with third parties so that you are not contacted if you ask not to be;
- Compile anonymous statistical information and analysis, which may be provided to Affiliates or to vendors or contractors of CCFMG;
- Make telephone calls or send text messages to you with information or offers, provided that you consent to be communicated to in this manner as described further below;
- Provide some or all of your information to Affiliates or other third parties for CCFMG own marketing purposes; or
- Permit you to access certain web-based programs or applications on the Site.
More Details About Information Collected For On-line Bill Pay
Certain of your information will be collected and used if you choose to pay your bill on-line from this website using the Online Patient Billing Manager. CCFMG is providing this service in partnership with Bank of the West. Categories of information collected by the CCFMG Online Patient Billing Manager may include identification, demographic information, and financial information. Specific personally identifiable information collected by CCFMG may include identifying patient name, patient Medical Record Number, your CCFMG Billing Account Number (used for patient identification and authentication), and your credit/debit card information (used to process payments). The collection and use of this information is governed by both State and Federal laws.
Additionally, CCFMG’s Online Patient Billing Manager web server may collect, at least temporarily, the following information: Internet Protocol (IP) address of the computer that is being used; web pages requested; referring web page; the browser that is being used; and the date and time. This information is collected to monitor your navigation of CCFMG web pages. CCFMG may use browser-IP-address information and anonymous-browser history to report information about site accesses and for profiling purposes. This information is generally used to improve web presentation and utilization. CCFMG also may use IP address information for troubleshooting purposes.
CCFMG’s Online Patient Billing Manager will not disclose or share personally identifiable information for any other purposes outside of payment without your authorization, except when disclosure is required by law. Your personally identifiable information will not be distributed or sold to third-party organizations.
Disclosure or Modification of Collected Information
You may request the disclosure of any or all personal information collected and stored by CCFMG about you. You may request the personal information CCFMG has collected and stored about you by filling out this form, Consumer Rights Request, and selecting the “Access My Information” option.
You may also contact CCFMG to modify, correct, or delete some or all of your information by modifying your information on the Site, by filling out this form, Consumer Rights Request, and selecting the “Delete My Information” option.
For any request to disclose, modify, or delete your information, CCFMG will take reasonable steps to verify your identity prior to disclosing or changing your information, such as requesting verification of other information you have previously provided to CCFMG, verifying information CCFMG have provided to you such as an account number, or by matching your means of connecting to the Site to similar information CCFMG previously collected.
Alternative Choice
Individuals who wish to use methods other than online enrollment may submit requests by phone or U.S. mail to the address provided on your paper billing statement.
Authorized Agent
You may designate an authorized agent to act on your behalf by sending a letter to the postal address below with a signed letter that includes your name, email address, and date of birth for verification, along with either your signed designation of the authorized agent or a court order or similar legal document designating the authorized agent as acting for you (for example, a power of attorney or letters testamentary).
Notice of Right to Opt-out
You may opt-out from receiving future offers from CCFMG, including having your email address removed – unsubscribed – from email marketing lists or text message lists by:
- Following the opt-out instructions contained in the marketing communication to opt-out of email marketing or text message marketing;
- By completing the form on the Do Not Sell My Personal Information webpage; or
- By sending an opt-out request in writing to privacy@inspirehealth.org with the subject line “Request to Opt-Out” and a description in the body of the email of the marketing or other use of the information from which you would like to be opted-out.
If you receive offers or other communications from Affiliates and would like to opt-out from future communications from them, you must contact them directly. In general, it takes up to 10 days for an Opt-Out request to take effect.
CCFMG may request authentication or verification of your request to opt-out, for example by requesting that you reenter your email address or provide other information, by requiring your authorized agents to provide additional information and authorizing documents to confirm their authorization by you, or by sending a confirmation email to you which you must confirm prior to the Opt-Out request taking effect.
You may disable cookies, web beacons and other technological means placed by CCFMG by adjusting your browser or email client settings. You may also opt-out from the collection of online viewing data for analytics purposes performed by Google Analytics by going to Ad Settings under your Google account. As noted above, CCFMG may identify you by your email address, third party cookies, or other means, and may use these methods to tailor your experience on CCFMG Site if you visit CCFMG Site or one of CCFMG Affiliate’s sites in the future.
You may revoke your consent to receive marketing text messages from CCFMG and/or the persons named in the consent by indicating that you do not want to receive any further text marketing from CCFMG or the persons named in the consent.
Security of Your Information
CCFMG uses commercially reasonable efforts to prevent unauthorized access or disclosure, or accidental loss or destruction of your information. CCFMG encrypts health-related information and payment-related information, and may encrypt other information that it collects. Given the nature of the Internet, your information passes through entities that CCFMG are unable to control. Therefore, CCFMG cannot guarantee that CCFMG security measures or those of CCFMG Affiliates or third parties who access or transmit your information will prevent your information from being illegally accessed, stolen or altered.
Your California Privacy Rights
The California “Shine the Light” law permits California residents to request information regarding CCFMG’s disclosure of your information to third parties including the categories of information shared and a list of the names and addresses of third parties with whom the information was shared. If you are a California resident and would like a copy of this notice, please send an email request to privacy@ccfmg.org and identify that it is a “Shine the Light Request” in the subject line of the email, or by sending a letter with the request to:

Most browsers contain a “do-not-track” setting. If your browser is set to do not track, the Site may not respond in accordance with that setting so that, for example, we will not be able to access any cookies stored in your browser. Site Policy Regarding Children Under the Age of 18 and Non-US Residents
The Site is intended to comply with the Children’s Online Privacy Protection Act (“COPPA”). COPPA requires that web site operators never knowingly collect personally identifiable information from anyone under the age of 13 without prior verifiable parental consent. The Site is not intended for minors under the age of 18 and we do not knowingly collect or use personally identifiable information from minors under the age of 18 on this Site. Only adults age 18 or over are permitted to use the Site.
By using the Site, you agree to provide us with accurate information concerning your age or identity if we request it. You also agree not to assist minors under the age of 18 in accessing the Site or to attempt to contact children under 18 through the Site.
The Site is also not intended for persons that are not residents (legally or otherwise) in the U.S.
Policy Changes
This privacy policy has been created and approved by Central California Faculty Medical Group Board of Directors, and may be updated with five business days’ notice, with such notice posted until at least five business days after the revised privacy policy takes effect. Any material revisions to this privacy policy will be reasonably prominently posted on the Site, and will apply to information collected both before and after the revision to the extent permitted by law. Your use of the Site following any such revisions will be deemed your acceptance of such revisions. Users should periodically visit this page to review our current policies on our collection and use of information.
